Ontario Provincial Police said that on Saturday at around 11:55 a.m., emergency crews responded to the scene on County Road 27 between Flos Road 11 East and Baseline Road, which is north of Barrie.
Three vehicles were involved in the crash.
An 80-year-old man and a 77-year-old woman, both from Midland, died at the scene, police said.
Two other people were taken to a local hospital with non-life-threatening injuries and a third person was taken to a Toronto-area trauma centre, police said.
The road was closed for most of the day Saturday as officers investigated.
There is no word on what caused the collision.
