Ontario Provincial Police said emergency crews responded to the scene at around 7 a.m. on Highway 35 between Sandy Hook and Pontypool roads in Kawartha Lakes.
The crash involved two vehicles, police said.
Three people were taken to a local hospital with injuries that were reported to be minor.
One person was taken to a trauma centre in Toronto with life-threatening injuries, OPP said.
The highway was closed for several hours as officers investigated.
